Ambient Clinical Documentation

Ambient clinical documentation is software that records the natural conversation between a clinician and a patient and drafts the clinical note from it, without anyone dictating. The product that does this is usually called an ambient AI scribe. It produces a structured note, for example HPI, exam, and assessment and plan, which the clinician reviews and edits. It is distinct from dictation, where the clinician narrates the note, and from transcription, where recorded audio is turned into text word for word.

In practice the terms are used interchangeably. "Ambient clinical documentation" is the term health systems and the research literature use for the category; "ambient AI scribe" or "AI medical scribe" is the term clinicians use for the product. Both describe software that drafts the note from the visit conversation rather than from dictation.

Studies so far point that way, with variation by product. The Permanente Medical Group estimated nearly 16,000 hours of documentation time saved across 7,260 physicians in its first year (Kaiser Permanente Division of Research, 2025). In a randomized trial at UCLA, one product reduced time in notes by 9.5% compared with usual care and another showed no significant change (Lukac et al., NEJM AI, 2025).

Consent requirements depend on your state and your organization’s policy, and recording a patient encounter is a different question from documenting it.
